Follow these instructions to do a Bereavement Risk Assessment prior to death of the patient. 

1. On the Patient's dashboard, click Care Management > Create New Documentation > Bereavement Risk Assessment. Click green Select button. 
   
   
   
   
2. Complete all the sections in the form. The bereavement contact's information is entered in the General section.
