HospiceWorks Guide: Social Worker & Chaplain Workflows

Overview

This guide is a specialized resource for Social Workers and Chaplains to master their daily activities in HospiceWorks. Whether you are conducting an initial assessment, managing ongoing visits, or preparing for Interdisciplinary Group (IDG) meetings, this article provides a step-by-step roadmap to navigate your documentation efficiently while keeping patient care at the forefront.

Core Workflow Pillars

1. Navigating Your Schedule & Active Visits

Learn how to manage your daily and weekly tasks directly from the main dashboard.

  • Next Visit Links: Use the hyperlinked dates in the "Next Visit" column to jump directly into documentation for pre-scheduled appointments.
  • Patient Search: Locate newly admitted patients or those without scheduled visits via the "Patients" tab in the top navigation bar.
  • Notifications: Stay updated with real-time alerts whenever a new patient or visit is assigned to you.

2. Assessments & Custom Care Plans

HospiceWorks simplifies the transition from assessment to action.

  • Initial Assessments: Access Psychosocial and Spiritual assessments directly from the "Tasks to Complete" list for new admissions.
  • Template Integration: Utilize pre-loaded care plan templates (e.g., Bereavement, Caregiver Fatigue) to streamline your plan of care.
  • Customization: If you need to update or edit a template, you'll learn how to coordinate with your system administrator to ensure your documentation reflects your specific clinical style.

3. Documentation Management

Never worry about losing progress with the system’s robust saving features.

  • Auto-Save & User Activity: HospiceWorks automatically creates a draft as soon as you open a document. If you get interrupted, your work is safely stored in the "Drafts" section.
  • Drafts vs. Complete: Use the "Manage Documents" feature to view, edit, or delete any work-in-progress.
  • Ongoing Notes: Learn the "Care Management" shortcut to manually create "Social Service Visit Notes" or "Chaplain Notes" for unscheduled or PRN encounters.

4. Mastering the IDG Process

Collaboration is key in hospice care. This section covers your role in the Interdisciplinary Group meeting.

  • Preparing for Meetings: Access the IDG Comprehensive Plan of Care through the "Care Management" menu to review and prepare your notes before the meeting starts.
  • Electronic Signatures: A walkthrough of the "View and Sign" process, ensuring your clinical input is finalized and legally compliant with a few simple clicks at the bottom of the document.

  Best Practice: The Breadcrumb Method

Instead of using your browser's "Back" button, use the Breadcrumb Navigation (e.g., Patients > Owen B > Documentation) at the top of the screen. This ensures you stay within the secure portal and return to the correct patient dashboard every time.
