The Bereavement Module in HospiceWorks provides a centralized platform for managing post-care workflows, risk assessments, and grief support for patient families and community members. This guide walks you through accessing the Bereavement Dashboard, managing plans of care, completing documentation, and running bereavement reports.


1. Accessing the Bereavement Dashboard

  1. Log into HospiceWorks.

  2. From the main top navigation menu, hover over or click Hospice.

  3. Select Bereavement from the dropdown menu.

2. Navigating the Dashboard & Case Categories

The Bereavement Dashboard categorizes cases to streamline workflow management:

View Categories

  • Community: Tracks bereavement cases for community members not directly enrolled in primary hospice care.

  • Hospice: Tracks bereavement cases linked directly to hospice patients.

Status Tabs

  • Pending: Patients who are discharged due to death automatically transition to this tab to initiate bereavement screening.

  • Open: Active bereavement cases currently receiving ongoing support and follow-up.

  • Closed: Cases where bereavement care services have been completed.

Tip: Click the View button next to any recipient's name to open their profile and Plan of Care.

3. Managing the Bereavement Plan of Care

Once inside a profile, use the top tabs to manage care details:

Profile Tab

Displays key demographic and clinical risk information:

  • Veteran and Youth status flags.

  • Address and primary contact details.

  • Initial and Current Risk Level ratings.

  • Deceased details (Name, Date of Death, Place of Death).

Plan of Care Tab

Manage and update standardized care plans directly within the system:

  • Problems: Review identified risk factors (e.g., emotional instability, withdrawal from grief discussions, behavioral issues).

  • Goals: Toggle completed goals and add progress notes (e.g., "Managing transition through grief process", "Demonstrates healthy coping strategies").

  • Interventions: Track scheduled action items, due dates, and disposition notes.

4. Completing Bereavement Documentation

Select the Documentation tab within a profile to create or review clinical notes:

  • New Bereavement Risk Assessment: Click this button to initiate a structured evaluation of grief risk levels.

  • New Coordination of Care: File clinical notes and document interdisciplinary communication.

5. Running Bereavement Reports

To track overall program metrics or scheduled follow-ups:

  1. Navigate to Hospice > Reports from the top menu.

  2. Under the Patient Reports section, choose from:

    • Bereavement Report: Generates a full summary of active/inactive bereavement details.

    • Bereavement Weekly Follow-Up Report: Displays upcoming and overdue follow-up contacts.

  3. Apply filters as needed:

    • Date Ranges: Select custom From and To dates.

    • Type & Status: Filter by Hospice/Community or Active/Inactive.

    • Special Demographic Filters: Toggle Veteran Only or Youth Only.

  4. Click Generate to view or export the report.
