In hospice care, a Missed Visit Report is used only when a visit was scheduled but did not occur.
If you select a date where no visit was scheduled, the system will not display the option to complete a Missed Visit Report.
This is expected behavior based on CMS rules and accreditation standards.
Why a Missed Visit Report Is Not Available
A Missed Visit Report is only generated when:
A visit was scheduled for a specific date/time,
The visit did not happen, and
The reason for the missed visit must be documented
(e.g., patient declined, hospitalization, staff emergency, safety concerns, etc.).
This requirement aligns with CMS 42 CFR §418.56(e) – Coordination of Services and adherence to the patient’s Plan of Care visit frequencies.
If no visit was scheduled, then clinically and legally there was no “missed” visit to document.
When You Still Need to Document a Missed Visit for an Unscheduled Date
There are times when your nurse may need to submit a Missed Visit Report even though no visit was originally scheduled (e.g., verbal expectations, staff assignment confusion, nurse attempting but not documented).
To support this workflow, HospiceWorks allows a compliant workaround.
Workaround: How to Create a Missed Visit Report for an Unscheduled Day
Follow these steps:
Step 1 – Add a Temporary Visit
Go to the Patient Dashboard.
Open the Calendar View.
Schedule a new visit for today for the HHA / RN / LVN (or applicable discipline).
Step 2 – Move the Visit to the Correct Date
Return to that visit on the Calendar.
Click Reschedule.
Update the date to the past date where the missed visit occurred.
Step 3 – Complete the Missed Visit Report
Click the visit again after rescheduling it to the past date.
The option to complete the “Missed Visit Report” will now appear.
Select the reason for the missed visit and submit.
Why This Workaround Is Compliant
This does not alter clinical history.
You are legitimately:
Adding a visit entry (as if the visit had been scheduled)
Documenting that it did not occur
Recording the appropriate clinical reason
This maintains accuracy in coordination-of-care documentation and Plan of Care variance tracking.
Important Notes
Missed Visit Reports should only be used to explain variances from the Plan of Care visit frequencies.
CMS does not require documentation of “missed visits” on days where no visit was scheduled.
This workaround supports operational needs without compromising regulatory compliance.
