Home Dashboard and Patient Overview

Upon logging in, the home screen provides an immediate snapshot of your caseload and daily priorities.

  • Assigned Patients: Displays a list of all patients currently under your care.
  • Next Visit: Includes hyperlinks to scheduled visits. Clicking these takes you directly to the visit documentation.
  • Quick View: An icon available throughout the system that opens a Patient Face Sheet. This contains:
  • Vital signs and body measurements.
  • Allergies and risk factors.
  • Summary of the latest nursing notes.
  • Needs Attention: A color-coded system to track documentation status:
  • Red: Overdue visits or documents past the standard timeline.
  • Yellow: Within the acceptable 48-hour window.
  • Green: On time.


Communication and Notifications

Hospice Works uses an integrated system to keep the care team synchronized.

  • Internal Messaging: Indicated by an orange envelope icon next to a patient’s name. Click the icon to read patient-specific messages.
  • System Notifications: Triggered by important events like new patient assignments, IDG (Interdisciplinary Group) review requests, or signature requirements.
  • Delivery: Notifications can be sent via SMS (if enabled) or email based on your user settings.


Schedule and Document Management

Effective time management is centralized through the "Care" and "Manage" tabs.

  • Manage Schedule: Allows you to view your schedule by day, week, or month. You can add new patient visits or non-patient activities and delete them as needed.
  • Manage Documents: A central hub for all drafts and documents awaiting your signature.
  • Days Outstanding: This column tracks how long a document has been pending, helping you stay compliant with agency timelines.
  • Auto-Save Feature: The system automatically saves all documentation in real-time. If you lose internet connectivity, your progress remains safe in the Drafts section.


Clinical Workflow and Patient Management

Each patient record contains specific tabs for detailed clinical management.

Patient Categorization

The Patients tab organizes the census into three categories:

  1. Active: Patients currently receiving hospice care.
  2. Pre-Admission: Patients between referral and admission. Use this section for Hospice Criteria Reviews.
  3. Discharged: Patients who have left the service (defaults to the last 30 days).


The Patient Dashboard

When selecting an active patient, the dashboard provides access to:

  • Plan of Care (POC): Summary of POC, physician orders, and assignments for Aides or Volunteers.
  • Medications: View the medication list and Administration Record (MAR). E-prescribing is available if enabled.
  • DME/Supplies: Track durable medical equipment and supply orders.
  • Care Management: A dropdown menu used to create new documentation, physician orders, or manage certifications.


Quick View Toolbar

Located at the top of the patient record, these icons allow for rapid actions:

  • E-Faxing: Send any document via fax directly from the system.
  • HOPE Records: View completed or pending admission, HUV1/HUV2, and discharge assessments.
  • Assignments: Quick list of all care providers (Physicians, Social Workers, Chaplains) assigned to the patient.
