Hospice Aide Workflow


Overview

This guide provides an end-to-end look at managing patient care, documentation, and scheduling within the Hospice Works platform.


1. Navigating the Dashboard

The dashboard is your primary hub for daily tasks. Key areas include:

  • Assigned Patients: A real-time list of patients under your care.
  • Patient Quickview: Click the small icon next to a patient’s MRN (Medical Record Number) to open a "face sheet." This displays contact info, vital signs, and the most recent documentation summary from the skilled nurse.
  • Patient Dashboard: Clicking the MRN number itself takes you to the full patient profile.
  • Daily Visits: In the "Next Visit" column, links for today’s visits are active. Clicking these takes you directly into the visit note.
  • Note: Visits scheduled for future dates will not be clickable until the actual day of the visit.

2. Managing Visit Documentation

When you enter a Hospice Aide Visit Note, the system displays assignment details such as vitals, bath assignments, and grooming tasks.

  • Auto-Save Feature: Hospice Works automatically saves your documentation as you work. There is no manual "Save" button. If you lose power or internet, your progress is secure.
  • Needs Attention Status: On your dashboard, this column tracks your progress:
  • On Time: Documentation is complete.
  • Draft Document: A reminder that a visit note has been started but not yet finished.

3. Messaging & Notifications

Stay connected with your care team through integrated communication tools:

  • Patient Messaging: Access patient-specific notes from the care team by clicking the envelope icon. These are not peer-to-peer chats but critical updates regarding patient care.
  • Alerts: You may receive notifications via email or SMS (if enabled) when a new communication note is available.
  • System Notifications: Located in the top-left, this center alerts you to new patient assignments or canceled visits.

4. Schedule & Document Management

Use the top navigation bar to manage your workload:

  • Manage Schedule: View your weekly or monthly calendar and filter visits by day.
  • Manage Documents: Access all "In Progress" or "Draft" documents. You can click Edit to resume work on any unfinished notes.

5. Managing the Full Patient List

To find patients not currently assigned to you or to look up past records:

  • Navigate to the Patients tab in the top bar.
  • Active/Pre-Admission/Discharged: Switch between tabs to find the relevant group.
  • Discharged Patients: This list defaults to the last 30 days. Clear the filter to view older records.

6. Starting an Unscheduled Visit

If you need to document a visit that wasn't on your calendar:

  1. Search for the patient and click their MRN.
  2. Click the Care Management dropdown.
  3. Select Create New Documentation.
  4. Choose Hospice Aide Visit Note and click Select.
