IDG Comprehensive Plan of Care: Workflow Guide

This guide outlines the steps for locating, completing, and finalizing the Interdisciplinary Group (IDG) Comprehensive Plan of Care (POC) within Hospice Works.


1. Accessing the Patient Dashboard

To begin the IDG process:

  • From your main dashboard, locate the patient in your Assigned Patients list.

  • Click on the MRN hyperlink associated with the patient. This will direct you to the specific Patient Dashboard.

2. Locating the IDG Dashboard

  • Once on the Patient Dashboard, click the Care Management dropdown tab.

  • Select IDG/Comprehensive POC from the list.

  • You will be taken to the IDG History page, which shows all scheduled and past meetings. Click Edit on the relevant meeting to open the form.

3. Setting Up the Meeting Details

  • Care Team: Scroll to the bottom of the form to assign the Attending Physician, Registered Nurse, Chaplain and Social Worker.

    • Note: If a member (like a Chaplain or Social Worker) is absent, select the appropriate reason from the dropdown menu.

  • Meeting Info: Return to the top of the form to enter the Start Time and select the IDG Category (e.g., Routine IDG, Admission, Discharge, etc.).

4. Documentation & Auto-Populated Data

Hospice Works uses an accordion-style system to organize data. Most sections auto-populate with information current up to the meeting’s effective date:

  • Demographics & POC Summary: Review patient status and functional assessments.

  • Assignments & Care Plans: Review active interventions and goals.

  • Medication Profile: Use the "click here to create a new one" shortcut to update medications or DME/Supplies via a Physician Order.

5. Documenting Narratives

Each discipline (Nursing, Chaplain, Social Worker) has a dedicated narrative section:

  • Click View Notes to see the documentation summary from the most recent visit.

  • Select Add to [Discipline] Notes to quickly pull that summary into the current IDG narrative for editing.

6. Finalizing Outcomes and Signatures

  • IDG Outcomes: Indicate whether the Plan of Care was updated.

    • If Yes, additional mandatory update fields will appear.

    • If No, the plan remains as currently documented.

  • Review: Click the Preview button at the bottom of the page to review the full document.

  • Locking: Once the review is complete, the RN must click Ready to Sign. This locks the document and sends it to all team members for electronic signature.
